Population Health & Outreach Coordinator

Population Health and Outreach Coordinator: Full-time position typically working Monday-Friday.

Major Position Responsibilities include:

• Develop and administer outreach programs and event coordination to manage and improve community relations
• Collect and analyze relevant data to identify areas of opportunity to improve the health of the community
• Oversee action plans to address the health needs and gaps in services that are identified

Qualifications:

• Bachelor’s degree in population health, business, a healthcare discipline or equivalent years of experience required
• Experience in healthcare, management, public relations, community health and wellness is preferred
• Computer knowledge required, with experience in Outlook, Excel, and Canva preferred
